High Value Inventory This is a sample of the High Value Inventory form included in the Pre-Move Information Packet. This document is used to identify items that you want to include in your household good shipment that are of extraordinary value. This allows Corrigan to be aware of items that need special care and attention.
Claim Form You should expect your goods to be transported and delivered in the same condition as they were at origin. However, in the unlikely event that you feel something was lost or damaged during the course of the move, please print out the attached Claim form, complete and return it to your Corrigan customer service representative for immediate processing. This form is for interstate household moves only. If your claim involves something other than interstate household goods transportation, please contact your Corrigan customer service representative for the correct claim form for that particular service.
Gypsy Moth Inspection Notification
U.S. Department of Agriculture regulations require outdoor household articles be inspected to prevent the transportation of gypsy moths from one state to another. The USDA “Don’t Move Gypsy Moth” booklet is provided in your Pre-Move Information Packet. Attached is the form that you should print, sign, and give to your van operator that verifies that your outdoor items are free from infestation.
Customer Service Survey
Customer feedback is very important to us, so we do ask that you take a few moments to complete the survey when you receive it. United Van Lines is committed to providing exceptional service, and therefore, view any score other than "5" as a failing grade. We ask you to understand that a score of "5" is not considered to be perfect - numerically it represents a score of 81 - 100 and should be viewed as "met expectations".
